bone Nebraska Animal Rescue Found Form bone

Name:
Email:
City: State:

Date the animal was found:

I have placed a "found" ad in the newspaper with no response.
I have checked the "lost" ads in the newspaper.
I have had this animal scanned and it has no microchip.
I have checked the "lost" information of my local Animal Control or humane society.
I have called local vets and reported I found this animal.
I have called radio stations and asked if they would announce this lost animal.
I have prepared and distributed flyers in the area where I found the animal.
I have contacted local rescue organizations to notify them so they can pass the word.

"Found" Information:

NOTE: When placing a Lost ad in the newspaper, we suggest you only put the breed of animal and whether male or female, no other details.  That way the 'true' owner can provide positive identification when they contact you. And only put your phone number in the ad and the general area where you found this animal.  Also, if you have a radio station announce you found this animal, hopefully they can be the contact and then provide the information to you so you can return calls on any possible leads and your personal information is not made public (for your own protection).

You will be unable to attach a photo to this form. If you need to send a photo, please send it to Nebraska Animal Rescue via an e-mail attachment.
